STUDENT HEALTH RECORDS

Conveniently located in the Student Service Center (SSC). Students can submit their immunization records for measles, mumps, rubella and a meningitis survey here. New York State Department of Health Laws #2165 and #2167 mandates that all incoming students born after December 31,1956 and taking 6 or more credits must show proof of vaccinations. Also, when transferring to a new school, students can obtain an immunization transcript. There is a $5.00 fee to be paid at the Bursar’s Office first. Present the paid receipt from the Bursar and you will be provided with an immunization transcript for you to send to your new school.
